It covers structured problem-solving frameworks such as 5 Whys, Fishbone (Ishikawa), Fault Tree Analysis, and Causal Factor Analysis, while emphasizing systemic thinking, human factors, and organizational learning. Participants learn to analyze failures, implement corrective and preventive actions, and apply RCA to improve process reliability, quality, and safety outcomes.

Day 2: Advanced RCA Techniques

  • Fault Tree Analysis (FTA) and Causal Factor Analysis
  • Process mapping and failure mode identification
  • Using RCA software tools and documentation methods
  • Case studies and practical exercises
Day 3: Implementation & Continuous Improvement

  • Designing effective corrective and preventive actions (CAPA)
  • Monitoring effectiveness and follow-up
  • Reporting, communication, and stakeholder engagement
  • Integrating RCA into quality, safety, and operational systems
  • Program review, lessons learned, and next steps

Assessment Methodology
