Course Description

Behavior-Based Safety (BBS) & Root Cause Analysis (RCA) is a practical and performance-driven training designed to help organizations reduce incidents by improving safety behaviors and strengthening investigation skills. This program combines proactive safety observation techniques (BBS) with structured incident investigation and RCA tools to identify real causes of unsafe acts, unsafe conditions, and system failures—leading to sustainable corrective and preventive actions.

Course Objectives

Upon the successful completion of this course, each participant will be able to:

  • Understand the principles and importance of Behavior-Based Safety (BBS) in preventing incidents.
  • Identify unsafe behaviors, unsafe conditions, and at-risk work practices.
  • Conduct effective safety observations and give constructive feedback.
  • Apply RCA tools to investigate incidents, near-misses, and unsafe events.
  • Distinguish between immediate causes and root/system causes.
  • Develop effective corrective and preventive actions (CAPA) that prevent recurrence.
  • Build a safety culture that encourages reporting, learning, and continuous improvement.
  • Integrate BBS findings into RCA investigations for stronger safety improvement outcomes.

Day 1: Introduction to Behavior-Based Safety (BBS) & Safety Culture

  • Overview of BBS and why it works
  • Safety culture vs safety compliance
  • Understanding the Human Factors in safety performance
  • Behavior vs attitude vs motivation
  • Unsafe acts and unsafe conditions (common workplace examples)
  • ABC Model (Antecedent–Behavior–Consequence)
  • Critical behaviors identification
  • Introduction to BBS programs (process and structure)
  • Workshop Activity: Identifying critical unsafe behaviors in participant workplaces

Day 2: Safety Observation, Communication & Intervention Skills

  • Conducting effective behavior observations
  • How to use BBS checklists and observation cards
  • Observation planning and selecting high-risk tasks
  • Identifying at-risk vs safe behaviors
  • Positive reinforcement and coaching techniques
  • How to approach workers without conflict
  • Effective communication techniques for safety conversations
  • Giving feedback and handling defensive reactions
  • Recording observations and reporting findings
  • Role Play Session: Worker observation and feedback simulation
  • Exercise: Create a customized BBS observation checklist

Day 3: Incident Investigation Fundamentals & Introduction to RCA

  • What is an incident? (accidents, near-misses, unsafe acts, property damage)
  • Importance of near-miss reporting
  • Incident investigation process and responsibilities
  • Evidence gathering: interviews, photos, site inspection, documentation
  • Immediate causes vs underlying causes vs root causes
  • Human error types (slips, lapses, violations)
  • Introduction to Root Cause Analysis (RCA) methodology
  • Building an investigation mindset: "Fix the system, not blame the person"
  • Case Study Exercise: Incident investigation practice with evidence review

Day 4: RCA Tools and Techniques (Practical Workshop Day)

  • The 5 Whys method (correct application and common mistakes)
  • Fishbone (Ishikawa) diagram for cause analysis
  • Fault Tree Analysis (FTA) basic introduction
  • Cause-and-Effect mapping
  • Barrier analysis and control failure identification
  • Identifying system weaknesses: procedures, training, supervision, equipment
  • Linking unsafe behavior to organizational/system causes
  • Writing strong problem statements and investigation reports
  • Workshop: Perform RCA using Fishbone + 5 Whys on a real incident scenario

Day 5: Corrective Actions, Prevention Planning & Integrating BBS with RCA

  • Developing effective corrective actions (short-term vs long-term)
  • CAPA development: corrective vs preventive actions
  • SMART action planning (Specific, Measurable, Achievable, Relevant, Time-bound)
  • Hierarchy of Controls (Elimination to PPE)
  • Monitoring and verifying action effectiveness
  • Trend analysis using BBS observation data
  • Integrating BBS data into RCA investigations
  • Building continuous improvement through safety leadership

Assessment Methodology

All courses conducted by EdTech will begin with a Pre-evaluation and end with a Post-evaluation.
